Washington State requires that all applicants for licensure of a home care or home health agency must submit a sample employee file and a sample client file.

While the employee sample file is the same for both types of agencies, the client file varies based on whether it is a non-skilled or a skilled sample. This file is constructed to meet the state requirements. Be sure to select the correct client file for your application, either Skilled or Non-skilled.
